MEDICAL WEIGHT MANAGEMENT

Before you choose care,
choose clarity.

Understand the process, the cost, and what happens next—so you can decide whether to take the next step.

For adults 18 and older. Availability depends on your location and the service. An evaluation does not guarantee a prescription.

Care options

Weight loss medications

Review the current product details and price. A clinician determines whether a prescription is appropriate for you.

HOW THE PROCESS WORKS

Know what happens,
and in what order.

01

Review the offer.

Read the product details, full price, included services, and any renewal or cancellation terms before paying. Choosing a product does not establish that it is appropriate for you.

02

Pay, then complete intake.

After checkout, continue to the designated clinical-intake process. Share your medical history there so a licensed clinician can evaluate whether treatment is appropriate.

03

A clinician decides.

Clinical care is provided by independent licensed clinicians. If the clinician decides not to prescribe, you receive a full refund. If treatment is prescribed, follow the care instructions provided to you.

THE COST, WITH CONTEXT

Look at the whole plan.
Not just the headline price.

The displayed price includes the consultation, prescribed medication, and shipping unless the order page expressly says otherwise. Read the details for the specific order before paying.

Laboratory work

Requirements depend on the medication and treatment plan and should be identified with the order. Additional blood panels are separate purchases.

Recurring charges

If an offer includes a subscription or membership, review its recurring price, billing interval, any minimum commitment, and cancellation steps at checkout.

IF NO PRESCRIPTION IS OFFERED

A full refund.

If the clinician decides not to prescribe, you receive a full refund.

BEFORE YOU DECIDE

A little more
clarity.

What happens if the clinician does not prescribe?

You receive a full refund. This applies when the clinician decides not to prescribe; it is not a promise of a particular treatment or outcome.

Am I paying before the evaluation?

Yes. Checkout comes before clinical intake in Reserve’s current website flow. Payment does not guarantee that a clinician will prescribe.

Who provides the medical care?

Independent licensed physicians and other clinicians through the medical group supporting the service. Reserve provides technology, care navigation, payment, and administrative support. The applicable provider’s identity and credentials are disclosed in connection with your care.

Can I ask about alternatives?

Yes. Ask your clinician about appropriate options, including approaches without medication, and the benefits, risks, and uncertainties of each. A preference or product selection does not determine what will be prescribed.

How do I cancel an automatic renewal?

For an automatic renewal, call or text +1 337-426-9520 or email care@reservelongevity.com before the medication order ships. Cancellation takes effect after Reserve confirms it. Orders already in pharmacy processing that cannot be stopped, or that have shipped, are generally final, except for the no-prescription refund policy and other stated exceptions. Review the current terms and your order’s details.

Where should I send medical questions?

Use the secure patient portal or the contact method provided by your medical group. For order or administrative questions, the published support contact is care@reservelongevity.com or +1 337-426-9520. Keep medical history out of marketing forms and ordinary email.

WHEN YOU’RE READY
